Prior to attending the workshop, participants of the program are required to conduct a chart review of five (5) self-selected patients with diabetes. The chart review will include several indicators of diabetes management. Six months following the workshop, participants must complete another chart review of five (5) self-selected patients with diabetes. Upon completion of the 6-month chart review and the Quality Improvement module available on-line at www.afmrd.org/qi, participants will be given a condensed slide set of the workshop curriculum. Completed chart review forms must be received in the AFMRD office two weeks prior to the workshop date you have chosen to attend. Completed forms may be faxed to 913.906.6105.
